In a groundbreaking study, researchers have unveiled findings that could change the landscape of hypertension treatment in India. The TOPSPIN trial, presented at the American Heart Association’s Scientific Sessions 2024, revealed that a combination pill incorporating two different blood pressure-lowering medications effectively reduced hypertension among Indian adults.
The Burden of Hypertension in India
With high blood pressure, or hypertension, affecting 300 million people in India and nearly half of U.S. adults, managing this health condition is critical. The results showed that participants taking any of the three medications experienced significant reductions in their blood pressure, achieving levels of 140/90 mm Hg or less.
Trial Design and Results
Participants in the trial were assigned to one of three groups, each receiving a combo pill containing two medications: the popular amlodipine and perindopril; or perindopril and indapamide; or amlodipine and indapamide. Remarkably, all combinations demonstrated equal effectiveness in lowering blood pressure over a six-month period.

Lead study author Dr. Dorairaj Prabhakaran, a cardiologist and executive director of the Centre for Chronic Disease Control in New Delhi, emphasized the importance of tailored treatments for the South Asian population. “India has an enormous burden of hypertension, and optimizing blood pressure management is essential to mitigate the risk of stroke and cardiovascular diseases,” he stated.
Participant Demographics and Outcomes
The trial included nearly 2,000 adults aged 30 to 79, both men and women, among which 55% were already on some form of blood pressure medication. During the study, the average reduction in blood pressure measured was substantial: approximately 30/14 mm Hg in a clinical setting and 14/8 mm Hg with 24-hour monitoring devices. Astonishingly, over 70% of participants reached blood pressure levels below the standard threshold, with more than 40% achieving even stricter targets.
Additional Health Benefits Observed
Additionally, participants receiving the amlodipine-perindopril combination showed a notable decrease in fasting blood sugar levels, suggesting added benefits for individuals with diabetes.
Study Limitations
Despite the positive outcomes, the study acknowledged some limitations, including that approximately 17% of participants did not complete the six-month regimen.

This study is particularly significant not just for India but also for the South Asian population in the U.S., which numbers over five million. The findings underscore the urgency of creating accessible treatment options as hypertension remains a prevalent health threat globally.
Call for Simplified Treatment Regimens
These promising results add weight to previous recommendations urging healthcare providers to pursue simplified treatment regimens for hypertension, such as the use of combo pills. According to the October 2021 American Heart Association scientific statement, using combination therapies may improve medication adherence and ultimately lead to better health outcomes for patients.
